APPLICATION REQUIREMENTS
An OPM applicant should be a Filipino citizen and should submit the following:

1. A filled-up and signed application form.
2. A presentable photograph (performance look).
3. A demo tape of at least 4 songs (tagalog & english).
4. OPM annual membership dues is only PhP850.00.
